Aren't those all very good reasons why insurance should be national?
Let me be a little more explicit: If we had one national set of insurance policies, then everybody in the country with a particular policy would pay the same premium. But health care costs in Oklahoma City might be 2/3 the cost of the same health care in Los Angeles or New York City. So the customers in Oklahoma City would be paying for for 2/3 X dollars worth of health care and the customers in LA would be paying
the very same premium for X dollars worth of health care. Hence the people of LA would be paying less than their share and the people of Oklahoma City would be paying more, thus subsidizing the folks on the coasts.
If you make state or regional premiums to fairly cover health care costs, then you have what we have now.
